CASE MANAGEMENT SERVICES:
A Case Manager will be assigned to each individual during admission, who will be responsible for co-ordinating the development and implementation of each Individual Plan of Care (IPC). The case manager will monitor the delivery of each aspect of the waiver services, as well as integrating various aspect of other services delivered under the waiver program and other sources. The case manager will keep accurate record of the individual's daily progress,goals and desires.
COUNSELLING & THERAPIES:
Our agency provides an array of counselling and therapy that include physical, occupational, speech/language therapies, as well as audiology and social work. We provide psychological services that include individualized plans designed to modify, develop and improve the cognitive, social and effective skills. Dietary therapy designed to meet individual nutritional needs are also provided.
RESIDENTIAL ASSISTANCE:
We offer residental assistance in the following categories:
- Supervised Living: Services provided to individuals by daily staff or by an individual staff member with periodic relief staff. Supervised living services is provided to consumers as needed in their residence and able to respond to the needs of the individual or individual during normal sleeping hours.
- Foster/Companion Care:
Services provided to individuals in a residence of a foster or companion care provider exclusive of Texas Department of Protective and Regulatory Services and Texas Department of Human services.
- Supported Home Living: Services to individuals residing in their own own residence or residence of their natural or adoptive parents.
RESPITE
Respite services are available to individuals residing in their own residence or residence of their natural or adoptive parents. Respite services may also include emergency and/or temporary residence services. Consumers may receive respite services up to 30 days each year.
(Residential Assistance Services Include):
Personal assistance with activities of daily living( grooming,feeding,bathing,dressing personal hygiene)
Meal planning and preparation
Securing and providing transportation
Support of independent living tasks including social interaction, appropriate behavior, liesure time and recreational activities
Assistance with medication
DAY HABILITATION:
Our agency assist the consumers to acquire, retain and improve self help, socialization and adaptive skills necessary to reside successfully in the community. We will also provide the consumers with age appropriate activities and recreation that enhance their self esteem and maximum fuctional level, complement and reinforce any counselling and therapy listed in the Individual Plan of Care (IPC).
SUPPORTED EMPLOYMENT:
Individuals under legal working age and determined by individual need may receive supported employment to facilitate community involvement.
NURSING :
Our agency provides nursing and dental services to ensure the general health and well-being of our consumers. Nursing include monitoring the individual's health and medical conditions, administration of medications and making referral and providing medical treatment as ordered by a physician.
DENTAL CARE:
Dental services include, emergency dental treatment,preventive, therapeutic and orthodontic dental treatments excluding cosmetic orthodontia.
ADAPTIVE AIDS:
Adaptive aids include devices,controls or appliances which enable persons to retain or to increase their abilities to perform activities of daily living or control the environment in which they live.
MINOR HOME MODIFICATION:
Minor home modifications include structural modification to the consumer's home to improve mobility or access depending on the individual's needs.
